Patients 12 and Under
The parents of patients aged up to 12 can request proxy access for their children and the parent would then be able to order repeat prescriptions online and book appointments online for their child, when the child turns 12 the surgery will require the patient as well as the parent / guardian to provide written consent by the time the patient turns 12 in order for access to continue.
Patients Aged Between 12 and 16
If the patient was registered via proxy access before they were 12, the surgery will request written consent from both the patient as well as the parent or guardian each year before the patient’s birthday until the age of 16, where access will be rescinded and the patient will need to complete another proxy access form if they do not have capacity or a SystmOnline registration form to allow them access.
If this is a new registration the patient can request access for themselves but will require a Gillick competency assessment prior to access being granted, or a proxy access form can be completed, but both patient and parent / guardian’s written consent but be present and this will be requested annually before the patient’s birthday, otherwise access will be declined.
