Although you will have a named accountable and allocated GP, all patients are free to see any clinician at the surgery. Although we appreciate that continuity of care is very important, it is not always possible to see the same GP due to the pressure on appointments. Your medical records are comprehensive allowing each clinician to look at your medical history in detail; seeing a different doctor for an ongoing problem will therefore not 'disadvantage' you.
If you are unsure of who is your named Doctor please ask at reception.
New patients are allocated a Doctor on Registration.
Anyone living within the practice area is welcome to register with the practice.
If you are unsure, you can also contact reception to confirm.
If you change your name and address or telephone number please inform us as soon as possible.
We take new patients who live outside the practice area at the discretion of the surgery.
If you move out of the area but wish to remain with the surgery that this can be arranged at the discretion of the surgery.
If you live in our practice catchment area (see above) you can join our list. If you are registering your new baby please view the next section.
To register please visit the surgery with the following information:
- Your Medical Card. If you do not have a Medical Card please complete a GMS 1 form. The GMS 1 form can be downloaded here, or ask reception for a paper copy.
- New Patient Health Questionnaire
- Third Party Representation form
- Summary Care Record patient consent form
- Proof of ID (e.g. passport, driving licence or medical card)
- Proof of Address (e.g. utility bill or bank/credit card statement from the last four months, current tenancy agreement, government agency letter, higher education letter or a letter from a recognised employer).
Transfer of Your Records
Some practices will be able to transfer your records electronically to us. If this is not possible and you are undergoing treatment or are on regular medication, you will need to see one of our doctors to continue your care here.
You need to register your new baby at the practice as soon as possible. Therefore please complete a GMS1 Registration Form and send it to your surgery. Please ensure you provide your baby’s NHS Number which will be detailed in their Red Book. Once we have received the form we will register your baby as a patient at this Practice.
Once your baby is registered you'll need to book an appointment for a mother and baby post-natal check. This is a 50 minute appointment, 30 minutes for the baby check and 20 minutes for the post-natal. Your baby’s course of routine immunisations will start at 8 weeks and you should book this in good time, advisable once seen for their 6 week check. Please remember to bring the red book. If you cannot make this appointment you must notify reception as soon as possible. Please be advised that if you do need to cancel, the next available appointment may be anything up to 4 weeks after that date.

If you are new to the UK and have not previously registered at another General Practice you will need to provide photographic identification when registering so that practice staff can request a unique NHS number on your behalf.
Please can you ensure completion of:
Hand both documents in at the surgery with 2 forms of identification (at least one of these must be photo ID, e.g. passport and/or driving licence + utility bill, birth certificate, bank statement) when registering.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply complete the Temporary Resident Registration Form (see below) and contact the practice.
Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
